Since 2022, Denmark has offered comprehensive genomic profiling to patients with incurable cancer in order to select targeted therapy. However, the evidence on clinical use of new targeted drugs remains sparse. Some are not yet commercially available through Named Patient Programs, while others have been rejected or not yet evaluated by the Danish Medicines Council. Additionally, data is not systematically collected, and access to these drugs varies across the country.

To address these challenges, a Phase IV prospective, non-randomised clinical trial is being initiated to investigate effect, safety and financial implications of commercially available, genomically targeted drugs to patients with incurable cancer.

The trial aims to establish a national, standardised approach for the provision of genomically targeted drugs. It will also focus on the systematic collection of real-world data (RWD) to support potential regulatory approvals in Denmark while incorporating economic considerations. Additionally, the trial aims to contribute to the development of a national infrastructure for genomic medicine in oncology.

With support from DCCC, the initiative will move forward with developing the study protocol and planning funding and implementation within a national collaboration. According to the applicants, this coordinated approach increases the chances of a successful trial. Close cooperation between clinicians, researchers, and regulatory authorities is key.

Rectal cancer treatment has traditionally relied on surgery, often combined with preoperative chemotherapy and/or radiotherapy. However, surgery can lead to significant side effects, including permanent stomas, reduced quality of life, and an increased risk of low anterior resection syndrome (LARS). Previous studies have shown that high-dose radiotherapy can achieve high organ preservation rates and improve local disease control without significantly increasing toxicity.

This project aims to develop a platform trial that expands on this approach by including patients across all tumor stages (T1-T4) and integrating different treatment strategies, including radiotherapy, chemotherapy, and their combinations. The trial will focus on organ-preserving radiotherapy for patients at various stages of rectal cancer within a structured clinical protocol.

DCCC’s support will help establish national clinical consensus on the study’s design and implementation. It will also strengthen national integration and support the development of an infrastructure for broad patient inclusion.

Prehabilitation is a key element in cancer treatment and requires high attention and action in Denmark. This need is emphasised in the recommendations for Cancer Plan V. Despite its well-documented benefits, Denmark lags behind other countries in implementing structured prehabilitation. A shared understanding of its value and contribution to patient pathways is essential.

To address this, a day of knowledge sharing is organised, focusing on evidence-based practices, interdisciplinary collaboration, and organisational strategies. The symposium will enhance participants' understanding of how prehabilitation can be effectively implemented across sectors and specialties, while also providing inspiration for integrating Cancer Plan V’s recommendations.

The programme will include presentations on recent research, artificial intelligence in treatment pathways, the role of onco-geriatrics, and patient perspectives.

The symposium is organised by the Danish Society for Geriatric Oncology, COMPAS, and the Danish Society for Oncological and Palliative Physiotherapy. It will bring together leading experts, clinicians, and decision-makers, including internationally renowned researcher Daniel Santa Mina from the University of Toronto. The event provides a platform for discussing and shaping the future of prehabilitation.
